Important Insurance Benefit Notifications for 2010
Your medical and pharmacy benefits remain the same for 2010 until the next Collective Bargaining Agreement is approved. Your deductibles and coinsurance start over on January 1st, 2010. There are no changes in your medical and pharmacy benefit at this time.
There have been some recent changes made to your Dental and Vision Plan. The actual plan document changes can be found on the vision and dental sections.
The most significant change to your vision benefit is that you can only receive new frames for glasses every two years.
The most significant change to your dental benefit is that the deductibles are increasing from $50 per per person, $150.00 family - to $100 per person, $300 family.
The out of network fee schedule used for dental services has been updated to reflect a more current usual and customary allowed amount. You must remember that if you choose to use an out of network dentist, that you will be responsible for the amounts over usual and customary as well as your deductibles and coinsurance amounts.
It is recommended that you always use in-network providers for all of your medical and dental benefits.
Their is no vision network. You may access any provider of your choice. The benefits payable remain the same regardless of your choice. It is important to note that most vision providers will require you to pay up front and then you can submit your claims to me for reimbursement. Your representative can file the claims to UMR for you.
There is also no benefit difference when choosing an orthodontist. You will receive the same lifetime maximum benefit of $1500.00 per person.
